    Background:

    • Chronic ischemic heart disease is a prevalent condition associated with various arrhythmias.
    • Monomorphic ventricular extrasystoles are a common finding in cardiac patients.
    • Distinguishing between extrasystoles and parasystole is crucial for accurate diagnosis and management.

    Purpose of the Study:

    • To investigate the characteristics of monomorphic ventricular extrasystoles in patients with chronic ischemic heart disease.
    • To explore the relationship between extrasystoles and parasystole based on linkage interval variability.
    • To identify potential transitional forms between extrasystole and parasystole.

    Main Methods:

    • Continuous 24-hour ECG monitoring was performed on 300 patients with chronic ischemic heart disease.
    • Monomorphic ventricular extrasystoles (≥30/hour) were identified.
    • Linkage intervals between extrasystoles were analyzed for fixed (≤0.1 sec) versus variable (>0.1 sec) oscillations.

    Main Results:

    • 89 out of 300 patients (29.7%) presented with monomorphic ventricular extrasystoles.
    • A fixed linkage interval was observed in 58 patients (62% of those with extrasystoles).

    • Variable linkage intervals in extrasystoles may indicate parasystole or transitional forms.
    • The prevalence of transitional forms suggests a potential common pathophysiological origin for extrasystole and parasystole.
