    Background:

    Purpose of the Study:

    • To investigate the link between endotoxin absorption and renal dysfunction in obstructive jaundice.
    • To explore the potential therapeutic role of bile salts in preventing endotoxin absorption.

    Main Methods:

    • Studied 24 patients with obstructive jaundice, assessing portal and peripheral endotoxemia.
    • Measured endogenous creatinine clearance pre- and post-operatively.
    • Conducted animal experiments to evaluate bile salt administration's effect on endotoxin absorption.

    Main Results:

    • 16/24 jaundiced patients had portal endotoxemia, with 13 developing peripheral endotoxemia.
    • Endotoxemia correlated with elevated serum bilirubin (>8.5 mg/100 ml).
    • Patients with endotoxemia showed significant postoperative decrease in creatinine clearance; those without did not.

    Conclusions:

    • Endotoxin absorption from the gastrointestinal tract contributes to renal impairment in obstructive jaundice.
    • Absence of bile salts in the intestine facilitates endotoxin absorption.
    • Oral bile salt administration may prevent endotoxin absorption and subsequent renal dysfunction.
