Background:

Infective endocarditis (IE) commonly affects heart valves, but isolated pulmonary valve IE is rare.
Streptococcus bovis is an increasingly recognized cause of IE, often associated with gastrointestinal pathology.

Observation:

A 62-year-old male presented with prolonged fever of unknown origin, later diagnosed with isolated pulmonary valve endocarditis.
The patient had no typical risk factors like intravenous drug use or congenital heart disease.
Pulmonary embolism occurred during treatment, complicating the clinical course.

Findings:

The causative agent was identified as Streptococcus bovis.
The patient responded well to targeted antibiotic therapy.
No colonic neoplasia was found during the subsequent gastrointestinal evaluation, despite the known association of S. bovis with such conditions.
