    Summary

This study compared four depression detection methods in fibromyalgia patients. The Beck Depression Inventory (BDI) showed high sensitivity, while the adjusted BDI (BDI-A) offered the best specificity for diagnosing depression.

    Area of Science:

    • Psychiatry
    • Rheumatology
    • Medical Diagnostics

    Background:

    • Fibromyalgia is a chronic condition often accompanied by depression.
    • Accurate depression detection is crucial for effective fibromyalgia management.
    • Various screening tools exist, but their comparative performance in fibromyalgia is not well-established.

    Purpose of the Study:

    • To investigate the relationships among four distinct methods for detecting depression in patients diagnosed with fibromyalgia.
    • To compare the diagnostic accuracy, sensitivity, and specificity of these methods.

    Main Methods:

    • A cohort of 100 women diagnosed with fibromyalgia (mean age 43) participated.
    • Four depression assessment instruments were utilized: Computerized Diagnostic Interview Schedule (C-DIS), Beck Depression Inventory (BDI), adjusted BDI (BDI-A), and Minnesota Multiphasic Personality Inventory depression subscale (MMPI-D).

    Related Experiment Videos

  • Statistical analyses included chance-corrected concordance, sensitivity, specificity, and accuracy calculations.
  • Main Results:

    • The Computerized Diagnostic Interview Schedule (C-DIS) identified 22% and the adjusted BDI (BDI-A) identified 29% with major depression.
    • The Beck Depression Inventory (BDI) and MMPI-D reported higher prevalence rates of 55% and 44%, respectively.
    • All four methods demonstrated agreement significantly above chance.
    • The BDI exhibited the highest sensitivity, while the BDI-A showed the greatest specificity when compared against the C-DIS.

    Conclusions:

    • The Beck Depression Inventory (BDI) is a sensitive tool for detecting depression in fibromyalgia patients.
    • The adjusted Beck Depression Inventory (BDI-A) offers superior specificity in this patient population.
    • Clinicians should consider the varying sensitivity and specificity profiles of different depression screening tools when assessing fibromyalgia patients.
