Summary

Detrusor contraction strength (WF) before prostatectomy may predict persistent symptoms and post-operative flow rates. However, WF is not a reliable predictor of individual patient outcomes after surgery.

Background:

  • Prostatectomy outcomes can be unsatisfactory due to challenges in pre-operative obstruction assessment.
  • Conventional urodynamic criteria may not accurately identify men who will benefit from surgery.

Purpose of the Study:

  • To investigate the relationship between detrusor contraction strength (WF) and urodynamic/symptomatic findings before and after prostatectomy.
  • To assess if WF can predict clinical outcomes following the procedure.

Main Methods:

  • Urodynamic investigation and symptom scoring were performed on 129 men before and 6 months after prostatectomy.
  • Detrusor contraction strength (WF) was calculated pre- and post-operatively.

  • A high pre-operative WF correlated with higher post-operative urinary flow rates (P=0.003).
  • Overall subjective clinical outcome was not strongly associated with WF, and it did not outperform simple voiding pressure measurements in multivariate analysis.

Conclusions:

  • Detrusor contraction strength offers insight into bladder function and prostatectomy outcomes.
  • WF is not a reliable predictor of individual patient outcomes after prostatectomy.
