Summary

This study reports a rare case of lymphocytic hypophysitis in a postpartum woman, leading to adrenal insufficiency and hypercalcemia. The findings suggest a link between thyroid hormone action, glucocorticoid deficiency, and elevated calcium levels.

Area of Science:

  • Endocrinology
  • Reproductive Medicine
  • Oncology

Background:

  • Lymphocytic hypophysitis is a rare inflammatory condition affecting the pituitary gland.
  • It often presents in the postpartum period and can cause selective pituitary hormone deficiencies.

Observation:

  • A young woman developed lymphocytic hypophysitis postpartum, resulting in selective corticotroph failure and secondary adrenal insufficiency.
  • She also experienced transient hyperthyroidism due to thyroiditis and presented with hypercalcemia.

Findings:

  • This is the third reported case of hypercalcemia associated with lymphocytic hypophysitis.
  • The presentation included selective corticotroph failure and concurrent thyroiditis, a pattern seen in previous cases.
  • The study proposes that thyroid hormone action in the context of glucocorticoid deficiency drives calcium efflux from bone.

Implications:

  • Hypercalcemia is a potential, though rare, complication of lymphocytic hypophysitis, particularly with co-existing adrenal insufficiency and thyroiditis.
  • Understanding the mechanisms of hypercalcemia in this context may inform clinical management.
  • This case highlights the complex endocrine interplay in postpartum pituitary inflammation.
