Summary

Chlamydia trachomatis (CT) infection may increase the risk of acquiring human immunodeficiency virus (HIV). Active CT infection was observed before HIV seroconversion in some individuals, warranting further investigation into this interaction.

Background:

  • Co-infections with Chlamydia trachomatis (CT) and human immunodeficiency virus (HIV) are common, particularly among men who have sex with men (MHS).
  • Understanding the interplay between CT and HIV is crucial for public health strategies and clinical management.

Purpose of the Study:

  • To investigate the serological interactions between Chlamydia trachomatis (CT) and human immunodeficiency virus (HIV) in a cohort of male homosexuals (MHS).
  • To determine if CT infection influences the acquisition of HIV.

Main Methods:

  • Sequential serum samples from MHS over a 4-year period were analyzed for CT-specific IgG and IgA antibodies using an immunoperoxidase assay.
  • Participants were stratified into HIV-negative, HIV-positive, and HIV seroconverter groups.
  • Matched heterosexual controls were included for comparison.

Main Results:

  • High prevalence of anti-CT IgG and IgA antibodies was observed in both HIV-negative and HIV-positive MHS compared to controls.
  • No increase in CT antibodies was detected after HIV infection.
  • Active CT infection preceded HIV seroconversion in a significantly higher proportion of seroconverters (3/10) compared to HIV-negative MHS controls (5%, P < 0.025).

Conclusions:

  • CT infection is highly prevalent in MHS and does not appear to be exacerbated by HIV.
  • Evidence suggests that active CT infection may precede and potentially facilitate HIV acquisition in MHS.
  • Further research is warranted to elucidate the mechanisms underlying the potential link between CT and HIV acquisition.
