Summary

This case study highlights multifocal neuropathy with persistent conduction blocks, a rare condition affecting both peripheral nerves and the central nervous system. Steroid therapy showed positive effects in this patient.

Background:

  • Multifocal neuropathy with persistent conduction blocks (MNPB) is a rare neurological disorder.
  • It is characterized by progressive sensorimotor deficits and conduction abnormalities in peripheral nerves.

Observation:

  • A 27-year-old man with a history of optic neuritis developed asymmetric sensorimotor deficits and areflexia.
  • Electrophysiological studies indicated multifocal neuropathy with conduction blocks and central nervous system involvement.
  • Nerve biopsy revealed onion-bulb formations, and MRI showed white matter changes.

Findings:

  • The patient presented with MNPB associated with central nervous system demyelination.
  • Anti-GM1 antibodies were negative, and CSF showed mildly elevated protein.
  • Onion-bulb formations on sural nerve biopsy are characteristic of certain demyelinating neuropathies.
