Background:

  • Varicocele, a common cause of male infertility, often recurs due to collateral vessel formation.
  • Traditional surgical interventions carry risks and may not fully address recurrence.
  • Percutaneous embolization offers a less invasive alternative for varicocele treatment.

Purpose of the Study:

  • To evaluate the efficacy and safety of ethanol embolization in treating varicoceles.
  • To assess the recurrence rates of varicoceles treated with ethanol embolization.
  • To investigate the impact of ethanol embolization on spermatic parameters.

Main Methods:

  • Forty patients with varicocele underwent embolization using ethanol (0.5-4 ml) injected into the testicular vein.
  • Fifteen patients also received stainless steel coils in addition to ethanol embolization.

  • The catheter tip was positioned near the inguinal region for targeted embolization.
  • Main Results:

    • Immediate post-embolization, varicoceles reduced in size in 3 patients and disappeared in 37.
    • Recurrence was observed in only 2 patients, both embolized at a proximal site.
    • Spermatic data improved in 13 patients; no improvement was noted in one azoospermic patient.
    • One serious complication occurred in a patient receiving a high dose (15 ml) of ethanol.

    Conclusions:

    • Ethanol embolization is an effective and useful treatment for varicocele.
    • The procedure demonstrates a low recurrence rate when performed correctly.
    • Ethanol embolization appears to be a safe treatment option with careful dosage management.
