    Background:

    • Extracorporeal electropiezolithotripsy (EPL) is a minimally invasive technique for kidney stone (urolithiasis) management.
    • Anomalous kidney morphologies present unique challenges for stone treatment.
    • This study evaluates EPL outcomes in a cohort of patients with diverse renal anomalies.

    Observation:

    • The study included 54 patients with various kidney anomalies, including shield, double, dystopic, L-shaped, S-shaped, and sponge kidneys.
    • Extracorporeal EPL was performed on patients with anomalous upper urinary tracts.
    • Urine passage distal to the calculus was a key factor for successful EPL.

    Findings:

    • Anomalous kidney structures are not absolute contraindications for extracorporeal EPL if distal urine flow is satisfactory.
    • EPL is a recommended treatment for urolithiasis in sponge kidneys when stones are located in the pelvis or calyces.
    • Attempts at EPL for stones within the kidney parenchyma led to significant complications, including massive macrohematuria and pyelonephritic attacks.

    Implications:

    • Extracorporeal EPL can be a viable option for treating kidney stones in patients with specific renal anomalies.
    • Careful patient selection and consideration of stone location are crucial for optimizing EPL outcomes.
    • EPL should be avoided for stones located within the kidney parenchyma due to high complication risks.
