Summary

Transhepatic hemodialysis access provides a viable solution for patients with end-stage lupus nephritis when other options fail. This approach offers a reliable vascular access for long-term dialysis therapy.

Area of Science:

  • Nephrology
  • Vascular Surgery
  • Dialysis Access

Background:

  • Reliable vascular access is crucial for hemodialysis, yet its establishment and maintenance pose significant challenges.
  • Patients with end-stage lupus nephritis often have limited vascular access options, with peritoneal dialysis sometimes being unsuccessful.

Observation:

  • A patient with end-stage lupus nephritis, who failed peritoneal dialysis and had no other vascular access alternatives, underwent a transhepatic catheter insertion.
  • The vascular access device was replaced once due to poor blood flow and reinserted once due to infection.

Findings:

  • The transhepatic approach for vascular access insertion resulted in no bleeding or thrombotic complications.
  • The patient successfully received adequate dialytic therapy for over one year using this transhepatic vascular access.

Related Experiment Videos

Implications:

  • Transhepatic catheterization represents a potentially effective alternative vascular access method for hemodialysis patients with complex conditions.
  • This technique may improve long-term dialysis outcomes for patients facing challenges with conventional access methods.
