Submicroscopic crystals in osteoarthritic synovial fluids

Summary

Osteoarthritis (OA) synovial fluid often contains tiny calcium pyrophosphate dihydrate (CPPD) or basic calcium phosphate (BCP) crystals missed by standard microscopy. Crystal presence in OA is a continuous process, not an on-off event.

Background:

Synovial fluid (SF) analysis is crucial for diagnosing joint diseases.
Conventional microscopy often fails to detect microcrystals in osteoarthritis (OA).

Purpose of the Study:

To investigate the presence of sub-microscopic calcium phosphate crystals in OA synovial fluid.
To test the hypothesis that OA SF contains crystals undetectable by standard methods.

Main Methods:

SF samples from OA, rheumatoid arthritis (RA), and pseudogout patients were processed using enzyme/hypochlorite extraction.
Extracted materials were analyzed using analytical electron microscopy (AEM) and X-ray powder diffraction (XRD).

Mineral deposits, including calcium pyrophosphate dihydrate (CPPD) and basic calcium phosphates (BCP), were found in 11 of 12 OA SF samples.

Many detected crystals and clusters were below 100 nm, explaining their absence under conventional light microscopy.

BCP was also detected in one RA sample.

Conclusions:

OA synovial fluid frequently contains CPPD or BCP crystals that are too small or sparse for conventional detection.
Crystal deposition in OA is a continuous phenomenon, not an absolute presence or absence.
