Summary

Many unmarried men experience impotence, often misdiagnosed as psychogenic. This study highlights that primary organic impotence, particularly vasculogenic, is a distinct condition requiring specific diagnostic approaches.

Background:

  • Impotence in unmarried individuals is frequently attributed to psychogenic factors.

Purpose of the Study:

  • To investigate the clinical spectrum of impotence in unmarried patients.
  • To differentiate between psychogenic and primary organic impotence in this population.

Main Methods:

  • A study was conducted on 160 unmarried impotent patients.
  • Patients were classified into psychogenic (145) and primary organic (15) groups.
  • Audiovisual stimulation penogram (AVS-penogram) was utilized for diagnostic assessment.

Main Results:

  • 15 patients were diagnosed with primary organic impotence, characterized by a history of erectile dysfunction without trauma or disease, and abnormal AVS-penogram findings.
  • Only 66% of the psychogenic impotence group showed abnormal classification.
  • Vasculogenic factors were identified as the most common cause of primary organic impotence.

Conclusions:

  • Primary organic impotence is a specific disease entity in unmarried patients that should be actively considered.
  • Comprehensive diagnostic methods are crucial for accurate differentiation from psychogenic impotence.
