Summary

Performing combined anorectal procedures with lateral internal sphincterotomy (LIS) does not increase complications. This study found no significant difference in incontinence or other adverse events compared to LIS alone for chronic anal fissures.

Area of Science:

  • Colorectal Surgery
  • Gastroenterology
  • Surgical Outcomes

Background:

  • Chronic anal fissures are a common condition requiring surgical intervention.
  • Lateral internal sphincterotomy (LIS) is a standard treatment for anal fissures.
  • The safety of combining LIS with other anorectal procedures is not well-established.

Purpose of the Study:

  • To compare the complication rates of combined anorectal procedures with LIS versus LIS alone.
  • To evaluate the impact of combined procedures on anal function and postoperative outcomes.
  • To determine if concurrent anorectal surgeries increase risks following LIS.

Main Methods:

  • A retrospective study comparing 57 patients undergoing combined procedures (including LIS) with 57 age- and sex-matched controls receiving LIS alone.

Related Experiment Videos

  • Data collected on complications such as incontinence, bleeding, abscess, and fissure recurrence.
  • Anal function was assessed postoperatively.
  • Main Results:

    • No statistically significant difference in the incidence of temporary incontinence between the combined group (8.7%) and the control group (7%).
    • No permanent incontinence was observed in either group.
    • Similar rates of postoperative bleeding, pruritus ani, mucus discharge, abscess formation, fistulation, and fissure recurrence were noted in both groups.

    Conclusions:

    • Combining additional anorectal procedures with lateral internal sphincterotomy does not elevate the risk of postoperative complications.
    • The safety profile of LIS is maintained when performed concurrently with other anorectal surgeries.
    • Patients can undergo combined procedures without a statistically significant increase in adverse outcomes.
