Summary

A new Tannenbaum (TB) stent design without sideholes significantly reduced biliary endoprosthesis clogging compared to traditional pigtail (PT) stents. This innovation offers improved patency for patients with malignant biliary obstruction.

Area of Science:

  • Gastroenterology
  • Medical Devices
  • Surgical Innovation

Background:

  • Biliary endoprosthesis clogging is a long-standing challenge for endoscopists.
  • Existing plastic stents with sideholes are associated with an increased risk of clogging.

Purpose of the Study:

  • To compare the patency of a new Teflon straight stent without sideholes (Tannenbaum stent) against traditional Teflon pigtail stents with sideholes in patients with malignant distal biliary obstruction.

Main Methods:

  • A prospective, nonrandomized study involving 74 patients receiving pigtail (PT) stents and 55 patients receiving Tannenbaum (TB) stents.
  • Both stent types were 10 Fr Teflon, used for malignant distal biliary obstruction.
  • Successful stent insertion and reduction in serum bilirubin were primary procedural outcomes.

Related Experiment Videos

Main Results:

  • The Tannenbaum (TB) stent demonstrated significantly longer patency compared to the pigtail (PT) stent (p < 0.02).
  • Median survival was 100 days for PT stents and 130 days for TB stents.
  • The TB stent's performance was comparable to metallic self-expandable biliary stents.

Conclusions:

  • The Tannenbaum stent, lacking sideholes, shows significant promise in reducing biliary stent clogging and improving patency.
  • Further randomized controlled trials are needed to compare the TB stent with current plastic and metal stents.
