Background:

  • Early detection and monitoring of DR progression are crucial for effective management.
  • Photopic electroretinograms (ERGs) offer a functional assessment of retinal activity.

Purpose of the Study:

  • To evaluate the utility of photopic ERGs in assessing the severity of diabetic retinopathy.
  • To correlate photopic ERG parameters (implicit time, amplitude) with DR stages.
  • To determine if photopic ERG implicit time can serve as an objective biomarker for DR severity.

Main Methods:

  • Photopic electroretinograms (ERGs) were recorded in 119 diabetic eyes and 19 normal control eyes.
  • ERGs were elicited using red or white flashes under bright light conditions.

    • Significant delays in photopic ERG implicit time were observed in moderate NPDR, PPDR, and PDR compared to mild NPDR, no retinopathy, and normal eyes.
    • The delay in implicit time progressively increased with the severity of retinopathy from mild NPDR to PPDR.
    • Photopic ERG amplitude reduction was noted in PPDR and PDR, but the correlation with DR severity was less significant than for implicit time.

    Conclusions:

    • Photopic ERG implicit time is a sensitive indicator for objectively evaluating diabetic retinopathy severity from mild NPDR to PPDR.
    • This finding supports the use of photopic ERGs in clinical decision-making, particularly for considering retinal laser photocoagulation in relevant stages of DR.
