Background:
- The link between hyperthyroidism (Graves' disease, toxic nodular goiters) and thyroid cancer is not well-defined.
- Thyroid carcinoma incidence varies (0.3%–16.6%), with higher rates in toxic nodular goiters and endemic areas.
- Occult thyroid cancer is common, often undetected preoperatively by imaging or fine-needle aspiration.
Purpose of the Study:
- To investigate the incidence and outcomes of thyroid cancer coexisting with hyperthyroidism.
- To evaluate the impact of concomitant malignancy on the management of hyperthyroid patients.
Main Methods:
- Retrospective analysis of 11 patients with thyroid cancer and hyperthyroidism (Graves' disease or toxic nodular goiter) between 1979-1991.
- Review of pathological findings (papillary, follicular carcinoma) and surgical procedures (subtotal/total thyroidectomy, lymphadenectomy).
- Assessment of patient outcomes and follow-up duration (mean 91.8 months).
Main Results:
- Eight papillary and three follicular thyroid cancers were identified in 11 hyperthyroid patients.
- Malignancy was diagnosed intraoperatively (frozen section) in 2 patients and postoperatively (permanent section) in 9.

- Concomitant thyroid malignancy and hyperthyroidism are rare.
- The presence of cancer does not negatively impact prognosis in these patients.
- Standard surgical treatment (total thyroidectomy) for hyperthyroidism effectively manages coexisting thyroid cancer.
