Summary

Atrial flutter cycle length changes with atrial size. This study shows that increasing atrial volume lengthens flutter cycle length, while decreasing it shortens the cycle, impacting arrhythmia maintenance.

Area of Science:

  • Cardiology
  • Electrophysiology
  • Medical Research

Background:

  • Type I atrial flutter is hypothesized to be a macroreentrant circuit.
  • The cycle length of such circuits is expected to vary with anatomical dimensions.

Purpose of the Study:

  • To investigate the relationship between atrial dimensions and flutter cycle length in type I atrial flutter.
  • To determine if changes in atrial volume and pressure affect the electrophysiological characteristics of this arrhythmia.

Main Methods:

  • Flutter cycle length was measured in seven patients undergoing programmed stimulation for type I atrial flutter.
  • Atrial volume changes were induced using postural maneuvers (tilting, respiration) and pharmacological agents (nitroglycerin).
  • Right atrial volume was assessed using B-mode echocardiography.

Related Experiment Videos

Main Results:

  • Negative tilt (increased atrial pressure) led to a slight increase in right atrial size (17.8 to 18.3 cm²) and a lengthening of flutter cycle length (228 to 233 msec).
  • Positive tilt with nitrates (decreased atrial pressure) reduced atrial size (to 16.6 cm²) and shortened flutter cycle length (228 to 219 msec).
  • Respiratory maneuvers and Valsalva maneuver also demonstrated cycle length variations correlating with atrial size.

Conclusions:

  • A direct relationship exists between atrial volume and flutter cycle length in human type I atrial flutter.
  • Right heart preload and atrial size are crucial factors influencing the electrophysiological properties of type I atrial flutter.
  • Understanding this relationship is vital for comprehending arrhythmia maintenance and therapeutic responses.
