Summary

Stroke can cause a rare unilateral hand tremor that mimics primary writing tremor. This study details a case of such a tremor following a cerebral cortical infarct.

Background:

  • Hand tremor is an uncommon stroke symptom.
  • Writing tremor has not been previously linked to stroke.
  • Cerebral cortical infarcts are the focus of this study.

Observation:

  • A 67-year-old male presented with mild right hemiparesis.
  • MRI revealed a discrete cortical infarct in the left frontal lobe.
  • Post-recovery, he developed a right-hand tremor during writing and similar tasks.

Findings:

  • The tremor exclusively affected the right hand during specific motor activities.
  • The tremor persisted for 7 months despite temporary relief with clonazepam.
  • The patient had a history of mild essential tremor, but no prior writing tremor.

Implications:

  • This case suggests cortical infarction can induce a unilateral tremor resembling primary writing tremor.
  • The precise role of pre-existing essential tremor in this presentation requires further investigation.
  • This finding expands the known neurological manifestations of stroke.
