Summary

Irregular discharges are common in dual diagnosis units, with younger patients and those with antisocial personality disorder being more likely to leave early. Current programs may not adequately address these retention challenges.

Area of Science:

  • Psychiatry
  • Addiction Medicine
  • Clinical Psychology

Background:

  • Irregular discharges pose significant challenges in dual diagnosis treatment settings.
  • Previous research on irregular discharges has primarily focused on psychiatric or substance abuse units.
  • Understanding factors associated with irregular discharge is crucial for improving patient retention in dual diagnosis programs.

Purpose of the Study:

  • To examine demographic and clinical variables associated with irregular discharge from an acute dual diagnosis inpatient unit.
  • To identify predictors of early and unplanned patient departures.
  • To inform program development for enhanced patient retention.

Main Methods:

  • Retrospective review of 316 consecutive admissions to an acute dual diagnosis inpatient unit.

Related Experiment Videos

  • Analysis of demographic data, discharge diagnoses, and substance abuse history.
  • Statistical comparison between regularly and irregularly discharged patients.
  • Main Results:

    • 37.7% of patients experienced irregular discharge (AMA, administrative, elopement), with significantly shorter lengths of stay.
    • Younger age and a diagnosis of antisocial personality disorder were significantly associated with irregular discharge.
    • The attending psychiatrist also emerged as a significant factor; demographic variables and substance abuse details were not predictive.

    Conclusions:

    • Dual diagnosis units face unique challenges with patient retention, particularly concerning younger individuals and those with antisocial personality disorder.
    • Existing treatment models may not sufficiently address the complex needs leading to irregular discharges.
    • Further research and program development are warranted to improve patient retention and treatment completion in dual diagnosis settings.
