Background:

  • A rare variant of appendiceal carcinoid, goblet cell carcinoid (GCC), is underrecognized by general surgeons.
  • GCC exhibits a more aggressive behavior than classic appendiceal carcinoids.
  • This tumor necessitates a distinct surgical strategy.

Purpose of the Study:

  • To characterize the clinical presentation and outcomes of goblet cell carcinoid of the appendix.
  • To differentiate GCC from classic appendiceal carcinoids.
  • To inform surgical management strategies for GCC.

Main Methods:

  • A retrospective review of clinical data from 9 patients (7 female, 2 male) diagnosed with goblet cell carcinoid of the appendix between 1983 and 1993.
  • Data were collected from tumor registries across 10 hospitals.

Main Results:

  • Patients presented with either acute appendicitis-like symptoms or chronic pelvic mass symptoms.
  • Diagnosis was not suspected preoperatively or intraoperatively in any cases.
  • Four patients with localized tumors treated with appendectomy had favorable outcomes (disease-free survival 2-11 years).
  • Five patients with diffuse appendiceal involvement developed intra-abdominal metastases and had poor prognoses (4 deaths within 5 months to 2 years).

Conclusions:

  • Goblet cell carcinoids represent a distinct clinicopathologic entity with variable malignant potential.
  • Diffuse appendiceal involvement by GCC warrants aggressive surgical intervention as part of multimodal therapy.
