Area of Science:

  • Medical research
  • Pulmonology
  • Infectious diseases

Background:

  • Older tuberculosis studies used less potent chemotherapy, limiting their applicability today.
  • Newer trials on tuberculosis treatments are often small and require careful interpretation.

Purpose of the Study:

  • To evaluate the role of corticosteroids in managing various forms of tuberculosis.
  • To determine appropriate corticosteroid use alongside anti-tuberculosis chemotherapy.

Main Methods:

  • Review of existing literature on corticosteroid use in tuberculosis.
  • Analysis of findings from prospective, randomized placebo-controlled trials (though limited in size).

Main Results:

  • Prophylactic isoniazid is not indicated for Mantoux-positive or previously treated tuberculosis patients starting corticosteroids.
  • Corticosteroids can be beneficial in managing pericardial tuberculosis, atelectasis in endobronchial tuberculosis, and severe pulmonary tuberculosis.
  • Corticosteroids may aid in treating pleural disease with prolonged symptoms and advanced tuberculous meningitis (stages II and III).

Conclusions:

  • Corticosteroid use in tuberculosis management should be selective, not prophylactic.
  • Corticosteroids can be a valuable adjunct therapy in specific, severe tuberculosis presentations.
  • Careful consideration of patient stage and disease type is crucial when using corticosteroids in tuberculosis treatment.
