    Summary

Percutaneous Balloon Valvuloplasty (PBV) shows good results for mitral stenosis but is rarely used for aortic stenosis due to disappointing outcomes. PBV is an option for elderly patients with severe aortic stenosis facing high surgical risks.

    Area of Science:

    • Cardiology
    • Interventional Cardiology
    • Cardiac Surgery

    Background:

    • Percutaneous Balloon Valvuloplasty (PBV) was introduced in 1984 and 1986 as a therapeutic option for mitral and aortic stenosis.
    • This minimally invasive procedure aims to widen narrowed heart valves using a balloon catheter.

    Discussion:

    • PBV demonstrates favorable immediate and mid-term outcomes in mitral stenosis when performed by experienced teams, often serving as a primary treatment comparable to surgical commissurotomy.
    • In contrast, PBV for aortic stenosis yields disappointing results and is infrequently employed.
    • However, PBV remains a viable option for elderly patients with aortic stenosis who have prohibitively high risks associated with traditional surgery.

    Key Insights:

    • Percutaneous mitral valvuloplasty is a frequently used and effective treatment for mitral stenosis.
    • Percutaneous balloon valvuloplasty for aortic stenosis has limited efficacy and is generally not recommended.
    • PBV in aortic stenosis is reserved for select elderly patients with high surgical risk.

    Outlook:

    • Further research may explore refinements in PBV techniques for aortic stenosis to improve outcomes.
    • Comparative studies evaluating PBV against other treatment modalities for specific patient subgroups are warranted.
    • The role of PBV in valvular heart disease management will continue to evolve based on patient selection and procedural advancements.
