Gastric ulcer in the elderly

A Pilotto1

Summary

Gastric ulcer (GU) secretion patterns remain consistent with age. Elderly GU patients show increased glycoproteins and N-acetylneuraminic acid (NANA), while H2 blockers demonstrate high healing rates.

Area of Science:

  • Gastroenterology
  • Geriatric Medicine
  • Oncology

Background:

  • Peptic ulcer disease, particularly gastric ulcers (GU), affects individuals across the age spectrum.
  • Limited understanding exists regarding age-related alterations in the gastric mucosal barrier.
  • Previous research indicates reduced prostaglandin levels in the gastric mucosa of elderly individuals.

Purpose of the Study:

  • To investigate age-related changes in gastric secretion and mucosal components in patients with peptic ulcers.
  • To compare secretory patterns between gastric ulcer (GU) and duodenal ulcer (DU) patients across different age groups.
  • To review the efficacy of H2 blockers in treating acute and maintaining chronic GU in the elderly.

Main Methods:

  • Analysis of basal and pentagastrin-stimulated acid, pepsin, pepsinogen group A, and gastrin secretion.
  • Measurement of prostaglandin levels, neutral glycoproteins, and N-acetylneuraminic acid (NANA) in gastric mucosa.
  • Review of published data on H2 blocker treatment for GU in elderly patients.

Main Results:

  • Gastric secretion patterns (acid, pepsin, pepsinogen A, gastrin) did not significantly change with age in GU patients.
  • Elderly GU patients exhibited higher concentrations of neutral glycoproteins and NANA compared to younger adults.
  • H2 blockers (famotidine, ranitidine, nizatidine) showed high healing rates for acute GU (67.6%–96.0%) and low relapse rates for chronic GU (8.29%–34.4%) in the elderly.

Conclusions:

  • Age does not significantly alter gastric secretion in GU patients, though mucosal barrier components like glycoproteins and NANA may increase in the elderly.
  • Further research is needed on factors like H. pylori, motility, hormones, and psychological aspects in elderly GU patients.
  • H2 blockers are effective for treating acute GU and maintaining remission in elderly patients.
