Surviving Boerhaave's syndrome without thoracotomy

Background:

Boerhaave's syndrome, a spontaneous esophageal rupture, is a critical surgical emergency.
Traditional management often involves thoracotomy, a major surgical procedure with significant morbidity.

Observation:

A patient presented with symptoms suggestive of esophageal perforation.
Diagnosis was delayed by 24 hours, with a barium swallow confirming a free rupture into the left pleural cavity.

Findings:

The patient with Boerhaave's syndrome and free pleural rupture was successfully managed without thoracotomy.
This represents a novel nonoperative approach for a condition typically requiring invasive surgery.

Implications:

This case suggests that nonoperative management may be a viable option for select Boerhaave's syndrome cases with free rupture.
Further research is warranted to explore the potential of minimally invasive or nonoperative strategies in esophageal emergencies.
