Summary

Hydroxyurea, a cytotoxic drug, can cause lung disease. A patient developed lung infiltrates and cavitation after hydroxyurea treatment, which resolved with discontinuation and corticosteroids.

Background:

  • Hydroxyurea is a cytotoxic agent used for various malignant and nonmalignant diseases.
  • It is generally well-tolerated, with myelosuppression as the main dose-related side effect.

Observation:

  • A patient presented with patchy interstitial infiltrates and cavitation two months into hydroxyurea therapy.
  • Pulmonary infiltrates resolved after discontinuing hydroxyurea and initiating empiric corticosteroid treatment.

Findings:

  • This case represents presumed hydroxyurea-induced lung disease.
  • Supportive lung biopsy material was obtained, making this the first such documented case.

Implications:

  • Hydroxyurea-induced lung disease is a potentially life-threatening toxicity.
