Summary

Acute polymyositis linked to Wuchereria bancrofti infection can cause severe muscle pain and weakness. Combination therapy with steroids and diethylcarbamazine effectively treated these cases, clearing microfilariae.

Background:

  • Polymyositis, an inflammatory myopathy, can present with muscle weakness and pain.
  • Wuchereria bancrofti is a parasitic nematode responsible for lymphatic filariasis.

Observation:

  • Two patients presented with acute polymyositis symptoms, including painful muscle swelling and weakness.
  • Diagnostic findings included elevated muscle enzymes, a myopathic EMG pattern, and inflammatory myopathy on biopsy.
  • Wuchereria bancrofti microfilariae were identified in the peripheral blood smears of both patients.

Findings:

  • Combination therapy using corticosteroids and diethylcarbamazine resulted in clinical improvement.
  • This combined treatment led to the total clearance of Wuchereria bancrofti microfilariae.
  • Steroid therapy alone was less effective compared to the combination regimen.

Implications:

  • This suggests a potential link between Wuchereria bancrofti infection and acute polymyositis.
  • Combination therapy may be a superior treatment strategy for polymyositis associated with W. bancrofti.
  • Further research is warranted to explore the pathogenesis and optimal management of this co-occurrence.
