Background:

Developmental dislocation of the hip (DDH) is a frequent congenital disorder affecting the infant musculoskeletal system.
Early detection and intervention are crucial for managing DDH and preventing long-term complications.
The natural progression of DDH is often unpredictable, highlighting the need for timely diagnosis.

Observation:

A case study of an 86-year-old male patient is presented.
The patient was diagnosed with asymptomatic bilateral developmental dislocation of the hip.
The diagnosis was incidental, discovered during unrelated medical evaluations.

Findings:

Bilateral developmental dislocation of the hip can remain asymptomatic into adulthood.
Late-onset or incidentally discovered DDH in elderly individuals is exceptionally rare.
This represents the second reported instance of incidentally diagnosed bilateral DDH in an elderly patient in orthopedic literature.

Implications:

This case challenges the typical presentation and progression of DDH.
It suggests that adult patients with undiagnosed DDH may present with subtle or absent symptoms.
Further research is warranted to understand the long-term outcomes and management strategies for incidentally discovered adult DDH.
