Background:

Neonatal acute leukemia is a rare and aggressive malignancy.
Rubella infection during pregnancy can lead to congenital abnormalities.

Observation:

An uncommon case of neonatal acute leukemia presented concurrently with serological evidence of rubella infection.
The infant exhibited an aggressive clinical course, succumbing to sepsis five days post-diagnosis.
Leukemic blasts displayed a mixed-lineage immunophenotype, co-expressing B-lymphoid and myeloid markers, along with progenitor cell markers.

Findings:

The leukemia cells showed co-expression of B-lymphoid (CD19, cytCD22, TdT) and myeloid (CD13, CD33, CD14, anti-MPO) markers.
Discordant expression of CD10 and CD34 epitopes was observed.

Karyotype analysis revealed a 46,XX t(4;11)(q21;q23) translocation, with HRX gene rearrangement at 11q23 confirmed.

Clonal biallelic rearrangement of the immunoglobulin heavy-chain gene was detected.

Implications:

This case suggests a potential link between rubella infection and the development of mixed-lineage neonatal leukemia.
The findings contribute to understanding the complex etiology of infant leukemias.
Further research is warranted to explore viral-induced oncogenesis in neonatal leukemia.
