    Summary

    Tropical sprue (TS) patients show impaired pteroylglutamic acid (PGA) absorption, particularly when glucose is present. This study demonstrates clear malabsorption using advanced perfusion techniques.

    Background:

    • Tropical sprue (TS) is a malabsorptive disorder affecting the small intestine.
    • Impaired nutrient absorption is a hallmark of TS, but specific vitamin absorption defects require further elucidation.
    • Pteroylglutamic acid (folic acid) is essential for cell growth and DNA synthesis.

    Purpose of the Study:

    • To quantitatively assess pteroylglutamic acid (PGA) absorption in untreated tropical sprue (TS) patients.
    • To compare PGA absorption in TS patients versus healthy controls.
    • To investigate the influence of luminal glucose on PGA absorption in TS.

    Main Methods:

    • Utilized a marker perfusion technique to measure PGA absorption.
    • Perfused physiological concentrations of PGA (25 ng/ml) in iso-osmotic solutions.
    • Solutions contained either mannitol or glucose (55.6 mM) and were administered to ten TS and eight control subjects on consecutive days.

    Main Results:

    • A statistically significant difference in PGA absorption was observed between TS and control subjects exclusively when glucose was present in the perfusate.
    • PGA absorption was demonstrably impaired in TS patients under specific conditions.

    Conclusions:

    • Unequivocal malabsorption of pteroylglutamic acid (PGA) is demonstrable in tropical sprue (TS) patients.
    • The presence of glucose in the perfusate is critical for revealing PGA malabsorption in TS.
    • Refined perfusion techniques can identify specific nutrient absorption defects in TS.
