A rare, slow-growing malignant tumor of the nail unit was identified in a 74-year-old female. This tumor, characterized by atypical keratinocytes and keratinous cysts, is proposed as a new entity: malignant proliferating onycholemmal cyst.

Area of Science:

  • Dermatopathology
  • Oncology
  • Nail Unit Neoplasms

Background:

  • Nail bed epithelium shares similarities with the outer root sheath of hair follicles.
  • Previous reports describe keratin cysts arising from nail bed epithelium.

Observation:

  • A slowly growing malignant tumor of the nail unit was observed in a 74-year-old female.
  • Microscopic examination revealed small keratinous cysts and nests of atypical keratinocytes invading the dermis and bone.

Findings:

  • The tumor exhibits features analogous to malignant proliferative trichilemmal cysts.
  • This entity is considered the malignant counterpart of previously described keratin cysts of the nail bed.

Implications:

  • Proposes a new classification for this specific nail unit malignancy.
