Summary

Childhood parapsoriasis, a rare condition, may precede mycosis fungoides. This study highlights the importance of immunophenotyping and long-term follow-up for pediatric parapsoriasis cases.

Area of Science:

  • Dermatology
  • Pediatric Oncology
  • Immunology

Background:

  • Parapsoriasis is infrequently reported in children, with some cases preceding early mycosis fungoides.
  • Understanding the evolution and potential links to mycosis fungoides in pediatric parapsoriasis is limited.

Observation:

  • Two pediatric patients (15-year-old boy, 8-year-old girl) with distinct parapsoriasis clinical features were monitored for four years.
  • Clinical, histological, immunophenotyping, and T cell receptor gene rearrangement analyses were conducted.

Findings:

  • Skin infiltrate composition differed: CD4+ and CD45+ lymphocytes in the first patient, CD8+ and CD45+ in the second.
  • No T cell receptor gene rearrangements were detected in either patient.

Implications:

  • The rarity and limited knowledge of childhood parapsoriasis underscore the critical need for long-term clinical and histological follow-up.
  • Immunophenotyping and immunogenotyping are crucial tools for characterizing pediatric parapsoriasis and assessing its relationship to mycosis fungoides.
