Area of Science:

  • Pulmonology
  • Rare Diseases
  • Pathology

Background:

  • Erdheim-Chester's disease is a rare systemic histiocytosis characterized by lipid-laden macrophages.
  • Visceral involvement, including pulmonary manifestations, can occur but is less common.

Observation:

  • A 50-year-old patient presented with chronic diffuse interstitial pneumonia.
  • Imaging revealed septal thickening, subpleural cysts, apical bullae, and pleural thickening.
  • Neurological involvement and bone/peri-aortic lesions were also noted.

Findings:

  • Respiratory function tests indicated a restrictive ventilatory defect with exercise-induced hypoxemia.
  • Bronchoalveolar lavage showed lymphocytosis and polymorphonuclear neutrophils.
  • Transbronchial lung biopsy confirmed Erdheim-Chester's disease with interstitial foamy histiocytes.

Implications:

  • This case highlights Erdheim-Chester's disease presenting primarily as interstitial pneumonia.
  • Early diagnosis and characteristic histopathological findings are crucial.
  • Stabilization of pulmonary symptoms without immunosuppressive therapy was observed, suggesting potential treatment avenues.
