  • Carotid cavernous sinus fistulas (CCSFs) can present with varied orbital symptoms.

Observation:

  • A young man presented with acute proptosis, chemosis, and vision loss.
  • Initial CT suggested orbital inflammatory pseudotumor, but treatment was ineffective.
  • Ocular motility was markedly limited with decreased visual acuity.

Findings:

  • Selective angiography revealed a dural cavernous sinus fistula.
  • Embolization of the fistula led to rapid resolution of proptosis and periorbital edema.
  • Final visual acuity improved to 20/200 in the affected eye.
