Summary

This study shows that systemic Amphotericin B effectively treats endogenous Candida endophthalmitis (ECE). Surgical intervention and intravitreal Amphotericin B are necessary for advanced cases, but visual recovery depends on lesion location.

Area of Science:

  • Ophthalmology
  • Mycology
  • Infectious Diseases

Context:

  • Candida endophthalmitis (CE) presents as endogenous ocular infection, often linked to systemic candidemia in immunocompromised individuals or as an isolated finding in intravenous drug users.
  • This study examines five cases of CE, including one with systemic candidiasis and four in drug abusers.

Purpose:

  • To evaluate treatment strategies and outcomes for endogenous Candida endophthalmitis (ECE).
  • To differentiate management approaches based on lesion confinement and extension within the eye.

Summary:

  • Systemic Amphotericin B was effective for treating localized retinal and choroidal lesions in ECE.
  • Cases with lesion extension beyond the internal limiting membrane required surgical removal of fibrovascular tissue and intravitreal Amphotericin B.

Related Experiment Videos

  • All patients showed lesion regression following treatment.
  • Impact:

    • Treatment strategies for endogenous Candida endophthalmitis can be tailored based on lesion characteristics.
    • Visual function prognosis is critically dependent on the location and extent of chorioretinal involvement post-treatment.
