Observation:

  • A 38-year-old male with a decade-long history of SVT experienced recurrence despite amiodarone treatment.
  • Ventricular fibrillation occurred after adding new antiarrhythmic drugs.
  • Electrophysiological studies confirmed orthodromic AV reentry tachycardia.

Findings:

  • Laboratory tests revealed a hyperthyroid state, exacerbated by amiodarone.
  • Discontinuation of amiodarone and introduction of methimazole led to symptom resolution within two months.
  • Amiodarone-induced hyperthyroidism can be subclinical or obscure.
