Background:

  • Acute pancreatitis is a rare but severe adverse effect of cancer chemotherapy.
  • Osteosarcoma is a malignant bone tumor often treated with chemotherapy.

Observation:

  • A 16-year-old female patient with metastatic osteosarcoma developed symptomatic pancreatitis.
  • Pancreatitis occurred recurrently within 24 hours of ifosfamide administration as a single agent.

Findings:

  • This is the first reported association between ifosfamide chemotherapy and acute pancreatitis.
  • Subclinical pancreatitis cases may be missed due to lack of routine serum amylase monitoring during chemotherapy.

Implications:

  • Clinicians should consider ifosfamide-induced pancreatitis in patients presenting with abdominal pain during chemotherapy.
  • Routine monitoring of pancreatic enzymes may be warranted for patients receiving ifosfamide or similar agents.
  • Further research is needed to elucidate the mechanism and incidence of ifosfamide-associated pancreatitis.
