    Summary

    Prednisolone treatment did not benefit patients with acute polyneuropathy, also known as Guillain-Barré syndrome. Steroid therapy showed no advantages and potentially worsened outcomes in this randomized trial.

    Background:

    • Acute polyneuropathy of undetermined aetiology, commonly Guillain-Barré syndrome, is a serious neurological condition.
    • The efficacy of steroid treatment in Guillain-Barré syndrome remains a subject of investigation.

    Purpose of the Study:

    • To evaluate the effectiveness of prednisolone in treating acute polyneuropathy of undetermined aetiology.
    • To compare outcomes between patients receiving prednisolone and those in a control group.

    Main Methods:

    • A multicentre, randomized trial involving 40 patients with acute polyneuropathy.
    • Patients were assigned to receive either prednisolone or no steroid treatment.
    • Neurological status was assessed using a six-point scale by blinded neurologists at one, three, and twelve months.

    Main Results:

    • The control group showed greater improvement than the prednisolone group at all assessment points.
    • A statistically significant improvement was observed in the control group compared to the prednisolone group at three months for patients treated within a week of illness onset (2.5 vs. 0.9 grades, P < 0.05).
    • The prednisolone group experienced more considerable disability (6 vs. 1) and relapses (3 vs. 0) compared to the control group. One death occurred in each group.

    Conclusions:

    • Prednisolone treatment is not beneficial for acute polyneuropathy of undetermined aetiology (Guillain-Barré syndrome).
    • Steroid therapy may be detrimental, leading to increased disability and relapses.
    • Current evidence suggests avoiding steroid treatment in this condition.
