Background:

  • Lyme disease, caused by Borrelia burgdorferi, is a tick-borne illness with diverse clinical manifestations.
  • Cardiac involvement in Lyme disease, such as Lyme carditis, can be severe and requires prompt recognition.

Observation:

  • A 62-year-old man presented with symptoms of Lyme pericarditis, rapidly progressing to cardiac tamponade and subsequent arthritis.
  • Diagnostic tests confirmed Borrelia burgdorferi infection in both pericardial fluid and synovial biopsies.

Findings:

  • Indirect immunofluorescence and monoclonal antibody immuno-gold silver stain identified Borrelia burgdorferi in pericardial fluid.
  • Silver staining revealed spirochetes in synovial biopsies, confirming disseminated infection.
  • The patient received intravenous ceftriaxone for 14 days for arthritis and underwent pericardiocentesis for tamponade.

Implications:

  • This case highlights the potential for Borrelia burgdorferi infection to cause life-threatening cardiac tamponade.
  • Prompt diagnosis and antibiotic treatment are essential for managing Lyme disease complications.
  • Further research into the cardiac manifestations of Lyme disease is warranted.
