    Summary

    Elevated serum thyroid-stimulating hormone (TSH) levels are sensitive indicators of thyroid failure in chronic thyroiditis patients, even with normal thyroid function tests. The TRH-test detects minimal thyroid failure, especially in asymptomatic autoimmune thyroiditis.

    Context:

    • Chronic thyroiditis often presents with elevated serum TSH, a key indicator of subclinical hypothyroidism.
    • Thyroid function tests in these patients may appear normal despite underlying thyroid dysfunction.

    Purpose:

    • To evaluate hypophyseo-thyroid functions in untreated euthyroid patients with chronic thyroiditis.
    • To assess the sensitivity of serum TSH levels and TRH-testing in detecting thyroid failure.

    Summary:

    • Elevated basal TSH levels, with a mean of 12.9 muU/ml, were observed in euthyroid chronic thyroiditis patients, often with low-normal T4 and high-normal T3 levels.
    • TSH responses to TRH injection were exaggerated in most patients, correlating with basal TSH levels and indicating minimal thyroid failure.
