Background:

  • Spontaneous rectus muscle hemorrhage is an uncommon condition.
  • It can present with sudden, unilateral, and painful proptosis.

Observation:

  • Three adult patients presented with spontaneous rectus muscle hemorrhage.
  • Clinical and CT findings were consistent with rectus muscle hemorrhage.
  • MRI confirmed recent hemorrhage in two patients.

Findings:

  • All three patients experienced spontaneous resolution of the hemorrhage.
  • No underlying cause was identified in these healthy adults.

Implications:

  • Spontaneous rectus muscle hemorrhage can occur without apparent cause in healthy individuals.
  • Observation is recommended in the absence of visual deterioration, as spontaneous resolution is expected.
