Alterations in erythropoiesis preceding leukemia

    Summary

Preleukemic changes in bone marrow show hypercellularity despite decreased erythrocyte production and longer erythroblast generation times. Patients with these bone marrow findings progressed to leukemia, indicating potential preleukemic alterations.

    Area of Science:

    • Hematology
    • Oncology
    • Cell Biology

    Background:

    • Preleukemic changes in bone marrow are poorly understood.
    • Hypercellular bone marrow with aregenerative anemia can precede leukemia.

    Purpose of the Study:

    • To investigate preleukemic changes in bone marrow cell proliferation.
    • To assess bone marrow kinetics in patients with hypercellular marrows and aregenerative anemia.

    Main Methods:

    • Studied seven patients with hypercellular marrows and aregenerative anemia.
    • Utilized 59Fe to estimate total bone marrow cell numbers, erythroblast generation times, and erythrocyte production.
    • Followed five patients to autopsy, several years after kinetic studies.

    Main Results:

    Related Experiment Videos

    • Despite bone marrow hypercellularity, total erythroblast numbers were not significantly increased.
    • Erythrocyte production and lifespan were decreased.
    • Erythroblast generation times were significantly longer than normal.
    • Five patients died with leukemia.

    Conclusions:

    • The observed bone marrow changes may represent preleukemic alterations.
    • Longer erythroblast generation times and decreased erythrocyte production in hypercellular marrows are associated with progression to leukemia.
