Summary

Osteogenesis imperfecta tarda can cause serious heart valve problems, leading to pulmonary edema. Routine echocardiography is recommended for connective tissue diseases to detect cardiac issues early.

Background:

  • Osteogenesis imperfecta tarda (OIT) is a rare genetic disorder affecting connective tissue, primarily bone.
  • Cardiovascular complications are not typically considered a primary manifestation of OIT.

Observation:

  • A 56-year-old woman with OIT presented with acute dyspnea and hemoptysis.
  • Chest radiography revealed pulmonary edema.
  • Initial transthoracic echocardiography suggested mitral valve chordal rupture and severe mitral regurgitation.

Findings:

  • Transesophageal echocardiography confirmed mitral valve pathology.
  • Histological examination of excised valve tissue showed acid mucopolysaccharides in the stroma, consistent with OIT.
  • The patient underwent successful mitral and aortic valve replacement.
