Background:

  • Upper gastrointestinal endoscopy (UGE) is a common procedure.
  • Sedation is often used during UGE to improve patient comfort.
  • Potential impacts of UGE and sedation on respiratory function are not fully understood in healthy individuals.

Purpose of the Study:

  • To evaluate changes in arterial blood gases during UGE.
  • To compare these changes in patients with and without midazolam sedation.
  • To assess respiratory effects in patients without cardiopulmonary disorders or smoking habits.

Main Methods:

  • A randomized trial involving 65 patients (16-59 years old) without cardiopulmonary issues or smoking habits.
  • Group I (n=40) underwent UGE without sedation; Group II (n=25) received i.v. midazolam (0.05 mg/kg) before UGE.

  • Arterial blood gases (PO2, PCO2, pH) and oxygen saturation were measured before, during, and after sedation (Group II).
  • Main Results:

    • Hypoxemia (decreased PO2 and oxygen saturation) occurred in 55% of non-sedated patients and 64% of sedated patients.
    • Sedated patients showed significantly greater decreases in PO2 (18.8 mmHg) and oxygen saturation (3.3%) compared to non-sedated patients (p < 0.001).
    • No significant changes in PCO2 or pH were observed; however, reduced chest expansion was noted in 70% of sedated patients.

    Conclusions:

    • Hypoxemia is a common occurrence during UGE, even in healthy, non-smoking adults.
    • Midazolam sedation exacerbates hypoxemia and may reduce respiratory excursion during UGE.
    • Arterial blood gas monitoring is recommended during UGE, particularly in sedated patients.
