Summary

Thin-section computed tomography (CT) is more sensitive for detecting beryllium disease than chest radiography. However, this imaging technique still misses the diagnosis in up to 25% of confirmed cases.

Background:

  • Beryllium disease, also known as chronic beryllium disease (CBD), is a granulomatous lung disease caused by beryllium exposure.
  • Accurate and early diagnosis of CBD is crucial for effective management and improving patient outcomes.
  • Radiographic imaging plays a role in diagnosis, but its sensitivity can be limited.

Purpose of the Study:

  • To detail the characteristic findings of beryllium disease on thin-section computed tomography (CT).
  • To compare the diagnostic sensitivity of thin-section CT with conventional chest radiography for beryllium disease.

Main Methods:

  • A study involving 28 patients with biopsy-proven beryllium disease and 12 healthy controls.
  • Chest radiographs were evaluated using an international classification system.
  • Thin-section CT scans were systematically assessed for 11 distinct findings affecting lung parenchyma, airways, pleura, and mediastinum.

Main Results:

  • Thin-section CT identified abnormalities in 89% of patients, compared to 54% with chest radiography.
  • Excluding smoking-related bronchial thickening, CT showed beryllium disease-specific changes in 75% of patients.
  • Common CT findings included parenchymal nodules (77%) and septal lines (71%).
  • Thin-section CT detected abnormalities in 77% of patients with normal chest radiographs.

Conclusions:

  • Thin-section CT demonstrates superior sensitivity compared to chest radiography for diagnosing beryllium disease.
  • Despite its improved sensitivity, thin-section CT may still fail to detect beryllium disease in up to 25% of histologically confirmed cases.
