Idiopathic drop attacks (DA) in an elderly woman were linked to severe hypothyroidism. Treatment with eltroxin resolved the falls, suggesting a direct relationship between hypothyroidism and drop attacks.

Area of Science:

  • Neurology
  • Endocrinology

Background:

  • Drop attacks (DA) are characterized by sudden, unwarned falls without loss of consciousness.
  • While symptomatic causes are known, the etiology of idiopathic DA remains unclear.

Observation:

  • A 70-year-old woman experienced a year of classic drop attacks.
  • Diagnostic evaluation revealed severe myxedema (hypothyroidism).

Findings:

  • Treatment with eltroxin led to complete clinical and endocrine recovery.
  • Drop attacks ceased with treatment and recurred upon discontinuation of hormone replacement therapy.

Implications:

  • This case suggests a direct correlation between hypothyroidism and the occurrence of drop attacks.
  • Hypothyroidism should be considered in the differential diagnosis of idiopathic drop attacks, particularly in the elderly.
