Summary

Scleral perforation from retrobulbar and peribulbar anesthesia is rare. This study found that even with perforation, surgery could proceed safely, with good visual outcomes and no need for further intervention in five reported cases.

Background:

  • Scleral perforation is an uncommon complication associated with retrobulbar and peribulbar anesthesia.
  • Most reported cases necessitate further medical or surgical intervention.

Purpose of the Study:

  • To report five cases of scleral perforation during retrobulbar and peribulbar anesthesia.
  • To evaluate the outcomes of surgical intervention following scleral perforation.

Main Methods:

  • Retrospective review of five cases of scleral perforation occurring between October 1991 and June 1992.
  • Surgical procedures were performed in all five cases despite the perforation.

Main Results:

  • All five surgeries were completed without intraoperative complications.
  • Vitreous hemorrhage was observed postoperatively in all patients.
  • No patient required additional intervention following the initial surgery.
  • Four patients achieved a final visual acuity of 6/9; one amblyopic eye had a final acuity of 6/18.

Conclusions:

  • Scleral perforation during retrobulbar and peribulbar anesthesia, while uncommon, does not necessarily preclude successful surgical outcomes.
  • Prompt surgical intervention can be safely performed, leading to favorable visual recovery even with associated vitreous hemorrhage.
