    Sodium caprylate dependent albumin auto-agglutinins can cause inaccurate Rh-hr blood typing results. Always use washed red blood cells for Rh-hr phenotyping when these auto-agglutinins are present in serum.

    Background:

    • Accurate Rh-hr phenotyping is critical for safe blood transfusions.
    • Certain auto-agglutinins can interfere with standard blood grouping procedures.
    • Albumin auto-agglutinins, specifically those dependent on sodium caprylate, are a known but often overlooked cause of serological discrepancies.

    Observation:

    • Three cases demonstrated erroneous Rh-hr phenotyping due to sodium caprylate dependent albumin auto-agglutinins.
    • Initial testing showed a negative albumin control in all presented cases.
    • An Rh0(D) negative individual's red blood cells were potentially misclassified as Rh0(D) positive.

    Findings:

    • The presence of sodium caprylate dependent albumin auto-agglutinins directly interferes with erythrocyte antigen detection during Rh-hr phenotyping.
    • Routine Rh-hr typing reagents from various manufacturers were evaluated, confirming the interference.
    • The auto-agglutinins caused false positive agglutination, mimicking the presence of Rh0(D) antigen.

    Implications:

    • Rh-hr phenotyping requires careful consideration of potential auto-agglutinin interference.
    • Washed red blood cell samples are essential for accurate Rh-hr typing when serum contains albumin auto-agglutinins.
