Area of Science:

  • Ophthalmology
  • Pathology

Background:

  • Orbital masses can present with proptosis, necessitating accurate diagnosis.
  • Lacrimal gland fossa lesions require careful evaluation.

Observation:

  • A 44-year-old man presented with progressive right eye proptosis.
  • Computed Tomography (CT) revealed a cystic mass in the right lacrimal gland fossa.

Findings:

  • Histological examination showed evidence of old and recent hemorrhages and chronic inflammation with foreign-body giant cells.
  • The lesion lacked epithelial lining, confirming the diagnosis of an orbital hematic cyst.

Implications:

  • Orbital hematic cysts are a rare but important differential diagnosis for proptosis.
  • Prompt diagnosis and surgical removal are crucial for managing orbital hematic cysts.
