Summary

Delayed-onset dystonia, a rare complication of stroke, can occur months to a year after brain injury. Lesion location in the basal ganglia appears crucial for developing this movement disorder.

Background:

Delayed-onset focal or segmental dystonia is an uncommon neurological sequela following cerebrovascular events.
The precise mechanisms and lesion locations contributing to post-stroke dystonia remain incompletely understood.

Observation:

This study details three cases of delayed-onset focal dystonia occurring after a stroke.
The time between the hypoxic brain injury and the onset of dystonia ranged from one month to one year.
Affected individuals included two adults and one child, all presenting with focal lesions in the contralateral basal ganglia.

Findings:

The latency period between brain damage and dystonia onset did not correlate with the patient's age at the time of hypoxic injury.
Focal lesions within the basal ganglia were consistently observed in the reported cases.

Implications:

The location of the brain lesion, specifically within the basal ganglia, may be a critical determinant in the pathogenesis of delayed-onset dystonia post-stroke.
Further research into specific neuroanatomical pathways is warranted to elucidate the mechanisms underlying this rare complication.
